Payroll Deduction members must submit the form by the 2nd and 17th of each month to prevent an additional payment from being withdrawn. AutoPay Memberships must submit the form by the 17th of each month to prevent being charged the next month’s payment. Failure to meet the dates above will result in the next payment being withdrawn.

The minimum refund request is $5. If your submitted cancellation is approved, refunds for all online purchases made within 6 months will be processed back directly to the original credit card used at purchase.
Personal Training: Refunds will only be considered with a Physician’s note indicating you can no longer participate in an exercise program. You can upload a copy of the note in the Request Form to expedite the request process.
Adventure Trip Refunds: If you would like to request a refund for any trip or clinic, this must be done within 5 business days before the trip. After that, we cannot guarantee any refunds.
